1. To save you time and provide professional and comprehensive assessment services, Dr.Foot foot assessments are by appointment only.

2. Fees for a single professional assessment: $300 for initial assessment if under 15 years old / $500 for assessment if 15 years old or older; $200 for reassessment.

3. The evaluation process will take approximately 30 to 50 minutes. Please allow ample time so that Dr. Foot can provide you with attentive and dedicated service.

4. Please wear loose clothing on the day of the assessment. If you are wearing long pants, pull them up to 10 centimeters above your knees, or bring shorts. Please also remember to bring your usual shoes for reference.

5. If you have a diagnosis or X-ray, please bring it with you for evaluation, so that the foot examiner can refer to it during the assessment.

6. The information you provide will not be made public or used for any other purpose, so please fill it out with confidence.
